Your records belong to you

You are entitled to copies of your consultation notes, investigation reports, discharge summary and bills. To request records, contact the reception or medical records desk with the patient's ID and visit details.

How to request

Requests can be made in person or by an authorised family member with the patient's consent. Please allow reasonable processing time for older records. Discharge summaries are normally handed over at discharge itself — keep them safely; they are the single most useful document at any future consultation, here or anywhere else.

Privacy of your records

Your medical information is confidential. Records are shared only with you, your authorised representative, or others as required by law. We do not share patient information with third parties for any other purpose.
